I understand a receipt can be emailed to me upon request and the charge will appear on my bank statement. I agree that no prior notification will be provided unless the date or amount changes, in which case I will receive notice prior to the payment.

I agree to provide a minimum of 15-day notice prior to cancellation of my membership.
