Membership Authorization Agreement, Cardholder Consent & Liability Waiver

By signing this document, I authorize Tunnel Baseball by Tunnel Athletics permission to process recurring charges to my credit card. I acknowledge and accept the membership terms, including the payment amount and the commitment period specified below.

I understand a receipt can be emailed to me upon request and the charge will appear on my bank statement. I agree that no prior notification will be provided unless the date or amount changes, in which case I will receive notice prior to the payment.

Following the initial commitment period, I agree to provide at least 15 days' written notice prior to cancellation of my membership.

I acknowledge and agree that participation in baseball training carries inherent risks of injury. By signing this contract, I release Tunnel Baseball from any and all liability for injuries sustained during training sessions.
